Required reporting for pesticide illness and injuries

We just posted updated reporting information and materials on our pesticides website [epi.dph.ncdhhs.gov]. This includes NEW provider memos – a one pager [epi.dph.ncdhhs.gov] that might be appropriate for posting on a clinic bulletin board and a longer version [epi.dph.ncdhhs.gov] with more details. The provider memos and website include a link and QR code to our NEW secure electronic reporting form [survey.dph.ncdhhs.gov] (yay no more faxes!). Please note that: Physicians are required to report pesticide illnesses and injuries [epi.dph.ncdhhs.gov]. Reports from physician assistants, nurse practitioners, and other medical providers are encouraged. Reports from exposed individuals, witnesses and other concerned citizens will also be accepted.
